Menopause and Natural Hormone Replacement Therapy

Understanding Menopause - Diagnostic Work Up For POF and Natural Menopause

Before making a diagnosis of POF the following conditions need to be ruled out since they are treatable:

Thyroid Disease
Hyperprolactinemia
Pituitary and/or Hypothamic Disorders
Cushing's Disease
Polycystic Ovarian Syndrome (PCOS)
Other causes of amennorhea (stopped periods) include: Excessive weight gain or weight loss (anorexia or bulimia); use of certain drugs (such as phenothiazines -- which are psychiatric drugs, and certain narcotics); excessive exercise; recent use of birth control pills (it's not uncommon to stop getting periods for up to six months after discontinuing the pill especially with Depro-vera. However, if your periods don't return after this time, this may not be related to the pill)


The two key ways to determine whether or not you're actually menopausal are

1) the length of time without a period, and
2) a test of your follicle stimulating hormone (FSH) hormone levels other tests which are helpful include serum estradiol level and the ratio of estradiol to estrone and HCG (pregnancy test) depending on age.

Blood work will demonstrate the following
FSH >30 consistently
Estradiol (E1) to Estrone (E2) ratio <1.0
Serum estradiol level <20
Negative pregnancy test (if indicated)
